Later physician updating information obtained during patient visit, morphing multiple or single part results in to a final personalized pictorial Medical (health) record. Personalized anatomical template pictures added serially over the prior visit documentations for the purpose of comparing to prior visit documentations in the same format, will help assess improvement or worsening of the condition.
