A method for personalizing a three dimensional health care record comprising : entering patient identification information, documenting symptoms and signs for multiple or single part of the body, updating information obtained during patient visit, morphing multiple or single part results in to a final personalized pictorial health record, personalizing three dimensional images to be added serially over the prior visit documentation results in time as the fourth dimensions in documentation of health record and providing security verification to update medical information.
