PROBLEM TO BE SOLVED : To provide a program for a health medical care cooperation system portable terminal device, a health medical care cooperation system, and a health medical care cooperation system integration base in which a user has a right to control his or her own information.SOLUTION : A program for a smartphone 30 owned by a user to be used in a health medical care cooperation system 1 causes the smartphone 30 to execute : a step that a two-dimensional code reading part 32 reads a two-dimensional code issued by any health medical institution among a plurality of health medical institutions; and a step that a linking approval setting part 33 requests identity authentication for linking the user and the health medical institution which has issued the two dimensional code and the smartphone 30 to the health medical care cooperation system 1, and when personal setting of whether to disclose at least health medical information of the user is input as for each of the plurality of health medical institutions, issues an instruction about whether to authorize the disclosure to the health medical care cooperation system 1.SELECTED DRAWING : Figure 3COPYRIGHT : (C)2018, JPO&INPIT
